The overhead press is the most commonly avoided compound movement after fifty — and the most commonly abandoned when shoulder discomfort appears. In most cases the discomfort is a technique problem, not an anatomy problem. Correct technique does not threaten the shoulder. It is one of the primary interventions that keeps it healthy.

The deadlift and the squat each have a dedicated technique page on this site. The overhead press — the third of the three fundamental compound movements most commonly performed incorrectly or abandoned prematurely after fifty — until now has not. This page provides it. It covers two variations: the standing barbell press, which is the most demanding shoulder health movement in the programme and the one that develops the stability and strength of the entire shoulder girdle most completely; and the seated dumbbell press, which is the more appropriate starting point for the over-50 trainee who has not pressed overhead before or who is managing a shoulder history that warrants a more conservative introduction.

The shoulder is the most mobile joint in the body and therefore the least inherently stable. The rotator cuff — the four small muscles that maintain the humeral head within the glenoid fossa — is the stability system that allows the shoulder's extraordinary mobility without the instability that mobility without support would produce. After fifty, the rotator cuff is more susceptible to impingement, partial tears, and tendinopathy than in earlier decades — not because the overhead press is inherently dangerous for the over-50 shoulder, but because the shoulder that has not been regularly trained through a full overhead range lacks the rotator cuff strength and the scapular stability that make pressing movements safe and productive.

The overhead press, performed correctly and progressed appropriately, builds exactly the rotator cuff strength and scapular stability that protects the shoulder from the impingement and tendinopathy that the untrained shoulder is most vulnerable to. The press is not the problem. The untrained shoulder that attempts to press without the foundation the press builds is the problem — and the solution is more pressing, done correctly, not less.

Scapular stability under load

The standing overhead press requires the scapula to rotate upwardly and stabilise against the pressing load — a demand that the bench press, which pins the scapula against a bench, cannot replicate. Scapular upward rotation under load is the specific motion that becomes impaired in shoulder impingement — the most common shoulder complaint in the over-50 population. Training the scapular stabilisers through the overhead press directly addresses the deficit that impingement produces and prevents.

Rotator cuff integration

The overhead press at moderate loads requires the rotator cuff to maintain the humeral head centred in the glenoid throughout the full pressing range — an integrative demand that isolation rotator cuff exercises, however valuable as prehabilitation, do not replicate under functional loading conditions. The rotator cuff strength built through correct overhead pressing is functional rather than isolated — the strength that transfers directly to the activities of daily life that require overhead reaching and carrying.

Overhead functional strength

The functional activities that require overhead strength — placing items on high shelves, lifting overhead, reaching across the body — are replicated most directly by the overhead press pattern. The bench press develops horizontal pushing strength; the overhead press develops vertical pushing strength. Both are required for complete upper body function, and the overhead pattern is the one most rapidly lost with age when it is not specifically trained.

Upper spine loading for bone density

The overhead press applies compressive loading to the cervical and upper thoracic spine through the muscle contractions of the upper trapezius and the weight of the pressing load — stimulating bone remodelling at the upper spinal segments that the deadlift and squat do not primarily load. For the over-50 trainee managing thoracic bone density, the overhead press is one of the few available exercises that specifically loads the upper spine under meaningful mechanical force.

The shoulder health argument — why pressing overhead protects the shoulder rather than threatening it

The shoulder complaints most common in the over-50 population are produced
by weakness and imbalance — the same deficits that progressive overhead pressing directly addresses.

Shoulder impingement — pain produced when the supraspinatus tendon is compressed between the humeral head and the acromion during overhead movement — is the most common shoulder complaint in the over-50 population. It is produced not by overhead movement per se but by the specific movement pattern of a shoulder with inadequate rotator cuff strength and scapular upward rotation that cannot maintain the subacromial space during elevation. The shoulder with a well-trained rotator cuff and adequate scapular stability maintains the subacromial space during overhead movement. The shoulder without this foundation does not.

Four shoulder health considerations specifically for the over-50 overhead press trainee

Each consideration addresses a specific aspect of shoulder health in the post-fifty context. Together they constitute the shoulder health framework within which overhead pressing is most safely developed.

Build pulling volume alongside pressing

The most common shoulder health error in any pressing programme is pressing volume that exceeds pulling volume — creating the anterior shoulder dominance and posterior shoulder weakness that produces the forward rounding posture most associated with shoulder impingement. The ratio of pulling to pressing should be at least one-to-one, and ideally two-to-one — two pulling movements for every pressing movement. For every overhead press session, at least one row and one vertical pull should be performed at equivalent or greater volume. The shoulder health of the over-50 trainee is determined by this balance as much as by pressing technique.

Include shoulder prehabilitation exercises

Band pull-aparts — fifteen to twenty repetitions at the start of every upper body session — activate the posterior rotator cuff and the lower and middle trapezius before any pressing load is applied. Face pulls with a cable or band — fifteen to twenty repetitions — reinforce the external rotation and scapular retraction that the rotator cuff requires to maintain the subacromial space during pressing. These two exercises cost five minutes per session and produce a disproportionate return in shoulder health maintenance for the over-50 trainee.

Stop below the pain arc

The painful arc of shoulder elevation — the range between approximately sixty and one hundred and twenty degrees of abduction where impingement typically produces pain — should not be trained through in the early weeks of overhead pressing. If pain is present in this range, begin with the seated dumbbell press at the pain-free range, build the rotator cuff strength through prehabilitation exercises, and reintroduce the full pressing range progressively as the pain arc reduces with the developing shoulder stability. Training through a painful arc reinforces the impingement mechanism it is working against.

Manage thoracic mobility alongside pressing

The overhead press requires thoracic extension — the ability of the upper back to extend slightly as the bar passes overhead — that the rounded thoracic posture common in the over-50 sedentary population does not naturally provide. Thoracic restriction forces the lumbar spine to hyperextend to compensate during the overhead press, creating the lower back arch that is the most common technical error and the most common source of lower back discomfort in the movement. Thoracic extension over a foam roller before pressing sessions addresses this restriction directly.

Variation one

Seated Dumbbell Press

The seated dumbbell press is the preferred starting point for the over-50 trainee beginning overhead pressing — because the dumbbells allow each arm to move independently, accommodating any asymmetry in shoulder mobility or strength without forcing the weaker or less mobile shoulder to match the stronger one. The seated position with back support reduces the core stability demand and the lumbar hyperextension risk that the standing press introduces, allowing the trainee to develop pressing strength and shoulder stability before adding the full-body coordination demand of the standing variation.

The seated dumbbell press also allows the press to begin from a position that avoids the painful arc if impingement symptoms are present — starting with the elbows below shoulder height and pressing to a range that is pain-free before progressively extending the range as shoulder health develops. This adjustability makes it the most appropriate first pressing variation for any over-50 trainee with a shoulder history, and a useful ongoing variation for any session where the standing press is not appropriate.

Seated dumbbell press — step by step setup and execution
  1. Sit on a bench with back support set to vertical or slightly inclined — ninety to ninety-five degrees. Hold a dumbbell in each hand at shoulder height, palms facing forward or at a slight angle inward. Elbows at approximately ninety degrees and slightly in front of the shoulder plane — not flared directly out to the side.
  2. Before pressing, retract the scapulae slightly — squeeze the shoulder blades gently together and downward. This scapular set position is maintained throughout the set and is the most important setup element for shoulder health in any pressing movement.
  3. Take a breath into the belly and brace the core — even seated, the core brace stabilises the torso and prevents the lower back from arching away from the bench as the dumbbells are pressed overhead.
  4. Press the dumbbells upward and very slightly inward — the natural arc of the dumbbell press brings the weights to a point where they nearly touch at the top of the range. This slight inward arc is correct and reflects the natural movement of the shoulder joint. Do not force the dumbbells to remain parallel throughout the press.
  5. At the top of the press, the arms should be fully extended with the dumbbells directly above the shoulders — not forward of the shoulders. If the dumbbells are forward of the shoulders at the top of the range, the thoracic mobility required for the true overhead position is not yet available. Stop at the available range and work on thoracic mobility.
  6. Lower the dumbbells under control — three to four seconds on the descent — returning to the starting position at shoulder height. The eccentric phase of the press is as important as the concentric for shoulder stability development. Never drop the dumbbells from the top position.
  7. Exhale on the press. Reset the core brace before the next repetition.
Variation two

Standing Barbell Press

The standing barbell press — pressing a barbell from the front rack position to a full overhead lockout — is the most demanding variation of the overhead press and the one that most completely develops the scapular stability, rotator cuff integration, and full-body pressing strength that the overhead pattern requires. The standing position means the legs, core, and hips must all contribute to the stability that the bench provides in the seated variation — making the movement a full-body exercise rather than an upper-body one, and developing the integrated strength that functional overhead activities require.

The standing barbell press should be introduced after the seated dumbbell press has established the pressing pattern and developed initial shoulder stability. The transition to the standing barbell press adds the thoracic mobility demand, the balance and core stability requirement, and the specific challenge of the barbell front rack position — all of which require the pressing foundation that the seated dumbbell variation builds. Begin the standing press at the empty bar and establish technique thoroughly before adding load.

Standing barbell press — step by step setup and execution
  1. Set the barbell in a rack at upper chest height. Grip the bar just outside shoulder width — slightly narrower than the squat grip — with the bar resting in the heel of the palm and the wrists directly above the forearms. The grip width that allows the elbows to be directly below the bar when it is at the shoulder position is correct.
  2. Unrack the bar and step back. Feet hip-width apart, a neutral stance. The bar rests in the front rack position — across the front deltoids, in contact with the clavicles, elbows pointing forward and upward. The bar should feel stable in this position before the press begins.
  3. Take a deep breath into the belly and brace the core hard. Squeeze the glutes. This full-body tension is the foundation of the standing press — without it, the lower back will arch to compensate for the pressing effort, which is both inefficient and a source of lower back discomfort in the movement.
  4. Press the bar upward — as the bar clears the forehead, move the head and torso slightly forward under the bar, allowing it to travel in a vertical path over the centre of mass rather than arcing forward. The bar path of the standing press should be as vertical as possible when viewed from the side.
  5. At the top of the press, the bar is directly above the head — over the base of the skull, not forward of it. The arms are fully extended, the elbows locked out, the shoulders elevated into a shrug position that ensures full shoulder elevation at the top of the range.
  6. Lower the bar under control — the same slight forward movement of the torso as the bar descends past the face, returning to the front rack position at the shoulder. Never lower the bar in a purely vertical path without the head and torso moving forward to allow it — this produces the bar hitting the face on the way down.
  7. Exhale at the top. Rebrace fully before the next repetition. For sets of more than three repetitions, rebrace between each rep — never complete multiple repetitions without the breath-and-brace that protects the spine throughout the press.
Common errors — the six mistakes most frequently made by over-50 trainees in the overhead press

Excessive lower back arch The most common standing press error — leaning the torso back as the bar is pressed, converting the vertical press into a partial incline press and loading the lumbar spine inappropriately.

Fix: Squeeze the glutes and brace the core hard before the press begins. The glute squeeze specifically prevents the anterior pelvic tilt that produces lumbar hyperextension. If back arch persists at the current load, reduce the weight — excessive back arch is almost always a load management issue, and reducing load while maintaining technique builds the foundation for returning to higher loads correctly.

Elbows flared too wide Elbows pointing directly out to the side in the starting position — externally rotating the shoulder to a position that increases subacromial impingement risk throughout the press range.

Fix: Bring the elbows slightly forward of the shoulder plane in the starting position — approximately thirty degrees forward of the direct lateral position. This slight forward angle reduces the impingement risk at the shoulder while maintaining the pressing efficiency of the movement. The cue is "elbows forward, not out" — a small adjustment that makes a significant difference to shoulder comfort in the pressing range.

Bar path drifting forward The bar travelling forward rather than vertically during the standing press — producing an inefficient pressing path and placing the load ahead of the centre of mass, which increases the moment arm and the spinal loading.

Fix: Move the head and torso slightly forward as the bar clears the face — allowing the bar to travel vertically while the body moves around it. The bar path is determined by the body's movement relative to it, not by attempting to push the bar in a specific direction. Practice the movement at empty bar weight to establish the body movement pattern before adding load.

Incomplete lockout at the top Stopping the press short of full arm extension and full shoulder elevation — missing the end-range position that develops scapular upward rotation under load and that represents the full training stimulus of the overhead press.

Fix: At the top of every repetition, ensure the arms are fully extended and the shoulders are actively elevated — a slight shrug at the top that drives the bar toward the ceiling. This active overhead lockout is the position that specifically trains the serratus anterior and upper trapezius that scapular upward rotation requires. Stopping short of this position leaves the most important part of the overhead press undertrained.

Wrists bent backward under load The wrist bending into extension under the pressing load — moving the bar out of the heel-of-palm position and into the fingers — reducing pressing efficiency and producing wrist discomfort that is often mistakenly attributed to the shoulder.

Fix: Position the bar in the heel of the palm — the area of the palm directly above the wrist joint — before unracking. Squeeze the bar hard throughout the set. The wrist should remain in a neutral or very slightly extended position. If wrist discomfort persists despite correct grip position, wrist wraps provide external support during the load-bearing position while wrist strength develops.

Pressing through shoulder pain Continuing to press when shoulder pain is present during the movement — reinforcing the impingement or rotator cuff irritation that the pain signals rather than addressing the technique or load issue producing it.

Fix: Stop pressing at any sign of sharp shoulder pain and assess the specific point in the range where it occurs. Pain in the arc between sixty and one hundred and twenty degrees of elevation suggests impingement — reduce range, reduce load, and increase prehabilitation volume. Pain at the top of the range suggests acromioclavicular joint irritation — reduce load and avoid maximum elevation until the irritation resolves. Never press through sharp joint pain.

Each adjustment addresses a specific challenge of pressing overhead after fifty. Together they constitute the approach that makes the overhead press a sustainable long-term movement rather than the exercise most commonly abandoned with age.

Adjustment 1
Begin with seated dumbbell press for four to six weeks

The over-50 trainee who has not pressed overhead before — or who is returning after a significant break — begins with the seated dumbbell press for four to six weeks before introducing the standing barbell press. The seated variation develops the pressing pattern, the rotator cuff integration, and the initial shoulder stability that the standing press demands without the full-body coordination requirement that makes the standing press more demanding to learn correctly. The transition to the standing press is made when the seated press is performed with full range and neutral technique at a challenging load.

Adjustment 2
Prehabilitation before every pressing session

Band pull-aparts and face pulls — fifteen to twenty repetitions of each — before every session that includes overhead pressing. These five minutes of prehabilitation activate the posterior rotator cuff and the lower and middle trapezius that maintain scapular stability throughout the press. The over-50 trainee who skips this preparation presses with a shoulder that has not been specifically prepared for the demands of the movement. The trainee who includes it consistently is building shoulder health with every session rather than simply managing the shoulder complaints that the unprepared shoulder produces.

Adjustment 3
Thoracic mobility work before every pressing session

Two minutes of thoracic extension over a foam roller — at three to four positions across the mid-back — before every session that includes the standing overhead press. The thoracic mobility that allows the bar to travel to a true overhead position above the head and behind the ears is frequently limited in the over-50 trainee. The five minutes of combined prehabilitation and thoracic mobility work before pressing sessions is the investment that makes the pressing sessions themselves consistently productive and pain-free rather than requiring constant technique adjustments as fatigue reduces the available thoracic extension.

Adjustment 4
Conservative loading — the overhead press progresses more slowly than the deadlift

The overhead press has the smallest absolute load of the three fundamental pressing and pulling movements — and progresses more slowly than any of them. Adding the smallest available increment — one to two and a half kilograms total — every two to three sessions when all reps are completed with full range and neutral technique is the correct progression rate for the over-50 overhead press. Attempting to progress at the rate appropriate for the deadlift produces shoulder overload in a joint that requires more conservative loading increments. The overhead press numbers in the training log grow slowly. They grow consistently. That is the correct outcome.

Adjustment 5
Use the landmine press as a shoulder-friendly alternative when needed

The landmine press — pressing a barbell anchored at the floor at a forty-five degree angle — provides a shoulder-loading movement in the partially overhead position that most over-50 shoulders tolerate even when the full overhead press is temporarily restricted by impingement, a rotator cuff history, or acromioclavicular joint discomfort. The forty-five degree pressing angle reduces the subacromial impingement risk while maintaining the pressing stimulus and the scapular stabilisation demand. The landmine press is not a permanent replacement for the overhead press but a productive alternative for the periods when the full overhead range is not available.

The shoulder that stops pressing overhead loses the rotator cuff strength and scapular stability that pressing overhead builds — and becomes more vulnerable to the impingement and tendinopathy that the press was preventing. The overhead press is not the exercise that ages the shoulder. It is the exercise that keeps it young. Learn it correctly. Progress it patiently. Press for decades.
